Payment
Payment is expected at the time the service is provided. Please make your payment in cash, check, credit card, or debit card. Visa, MasterCard, Discover, and American Express are all accepted.
Appointments
As a courtesy to other patients and employees, please plan to be on time for your appointment. Call us today to schedule your appointment.
Medication
Please bring your whole prescription (or a list of medications) to each session. This allows the doctor to go through allergies, instructions, side effects, prescription interactions, and doses.
Insurance
We will charge your insurance company as a courtesy if you supply us with the necessary paperwork. Copayments and deductibles must be paid in full at the time of service. Your agreement with the insurance carrier is between you and your insurance provider, and as such, we do not routinely investigate refused claims or claims that have not been paid in accordance with the contract. You will be liable for the unpaid balance if your insurance provider does not pay within 60 days of the date of the invoice. Any services performed that are not covered by your insurance plan must be paid in full at the time of your visit or when you get notice of a denied insurance claim.
Cancellations
Please be respectful of other patients and the doctor by providing a 24-hour notice if you plan to cancel your appointment. If you do not submit a 24-hour notice during the week, your account will be charged a minimum $50 "No Show or Late Cancel" fee.
Late Payment
Unless prior agreements have been made, a 2% service fee may be charged to any outstanding debt that is more than 60 days old.
Delinquent Accounts
If an account is not paid as requested, it will be turned over to a collection agency, and a 30% charge will be applied to the amount. Before any additional appointments may be booked, this payment must be paid in full.
